Open it, and under What do you need assistance with? choose Intellectual property requests.

Then, under What type of Intellectual Property Request do you need assistance with?, choose Copyright Infringement / DMCA.

The rest of the form is straightforward. You provide your email address, a subject line, and the details of your complaint. In the details, identify your copyrighted work, list the exact Reddit links where the infringing copies appear (the post or comment URLs, not just a subreddit name), and state that you own the rights and that the use is not authorized. You can attach supporting files if you have them, though that is optional. What happens after you submit
Once you send the form, Reddit follows up by email. You will receive a message confirming that the content has been removed, rejecting the claim, or asking for more information. If they ask for more, respond promptly with what they need, since a quick reply keeps the request moving.
